Battery-Free Hypoglossal Nerve Stimulation

🏆 Early Adopter in North Texas

Dream Sleep Medicine is among the first practices in North Texas offering Genio® bilateral hypoglossal nerve stimulation. Dr. Jain serves as National Faculty, training other physicians nationwide on advanced Genio implantation and titration techniques.

Genio uses a battery-free design where the implanted chip is powered wirelessly by a disposable adhesive patch worn on your neck during sleep. This design eliminates the need for surgical battery replacement procedures that would otherwise be required every 10-12 years with implanted battery systems.

Key Genio Features

Battery-Free Implant

The implant contains no battery. It is powered wirelessly by a disposable patch worn on the neck during sleep. This design eliminates surgical battery replacement procedures.

Single Incision

Procedure requires one small incision under the chin. All components are placed through this single access point.

Smartphone Control

Control your therapy and track usage directly from your smartphone via Bluetooth connectivity.

Bilateral Stimulation Advantage

Genio stimulates both the left and right branches of the hypoglossal nerve simultaneously, creating symmetric tongue movement. This bilateral approach offers several benefits:

  • Symmetric airway opening regardless of sleep position
  • Effective in all sleeping positions (back, side, stomach)
  • More natural tongue movement pattern during sleep
  • Potentially beneficial for patients with multi-directional collapse

Note: Both bilateral (Genio) and unilateral (Inspire) approaches are FDA-approved and clinically effective. The optimal choice depends on individual airway anatomy, which Dr. Jain evaluates during drug-induced sleep endoscopy (DISE).

How Genio Works

1

Apply the Patch

Before bed, apply a disposable adhesive patch to your neck (over the implant site). The patch contains the power source and wireless transmitter.

2

Activate via Smartphone

Use the Genio smartphone app to turn on the device. The patch wirelessly powers the implant and begins bilateral tongue stimulation.

3

Sleep Naturally

The implant gently stimulates both sides of the tongue throughout the night, keeping your airway open. Remove the patch in the morning.

The Science

Genio delivers continuous bilateral stimulation to the hypoglossal nerve (cranial nerve XII), which controls tongue movement. By activating both the left and right branches of this nerve, Genio prevents the tongue from collapsing backward during sleep-eliminating airway obstruction. The stimulation is mild, rhythmic, and typically imperceptible once you fall asleep.

Genio Implant System

Genio bilateral hypoglossal nerve stimulation implant system showing bilateral electrodes and battery-free chip

The Genio system uses bilateral stimulation electrodes placed through a single under-chin incision.

Activation Patch

Genio disposable adhesive patch worn on neck to wirelessly power the implant during sleep

The disposable patch wirelessly powers the implant each night - no battery replacement surgery needed.

The Genio Procedure

Single-Incision Surgery (90-120 Minutes)

Genio implantation is performed as an outpatient procedure under general anesthesia. The surgeon makes a single small incision under the chin (submental area) to place two tiny stimulation electrodes on the left and right branches of the hypoglossal nerve. The neurostimulator chip is also placed in the same pocket. No chest incision is required. Most patients go home the same day.

Recovery (1-2 Weeks)

Recovery is generally faster than Inspire due to the single-incision approach. Expect mild swelling under the chin, temporary tongue numbness, and soreness for 5-7 days. Most patients return to work within one week. Strenuous activity should be avoided for 2 weeks. Pain is typically managed with over-the-counter medications.

Activation (2 Months Post-Op)

After healing, you return to our clinic for device activation. We pair the implant with your smartphone app, provide patches, and gradually increase stimulation strength over several weeks. This "titration" period allows your body to adapt. Most patients reach therapeutic levels within 2-3 months.

Titration Sleep Study (3-6 Months)

Once comfortable with the device, we perform an in-lab titration study to optimize settings. Dr. Jain fine-tunes the stimulation parameters based on real-time sleep data, ensuring maximum efficacy. This advanced titration (which many practices cannot perform) is critical for optimal outcomes.

Dr. Jain's Perspective

"Both systems are highly effective FDA-approved therapies with comparable clinical outcomes. The choice depends on individual anatomy, lifestyle preferences, and patient priorities. Some patients prefer the battery-free design and single incision of Genio. Others prefer the breathing-synchronized stimulation and no external components of Inspire. As one of the few physicians trained on both systems, I provide objective guidance to help you choose what's best for your specific situation."

Clinical Evidence: DREAM Trial

Genio's FDA approval (August 2025) was based on the pivotal DREAM trial, which demonstrated robust clinical efficacy at 12-month follow-up:

  • 63.5% AHI responder rate (≥50% AHI reduction and AHI <20)
  • 70.8% median AHI reduction from baseline
  • 90% patient satisfaction - would recommend to others
  • Significant improvements in Epworth Sleepiness Scale and quality of life

The earlier BLAST OSA trial led to European CE Mark approval in 2019, with extensive real-world data from European patients. As an early adopter in North Texas, we've observed comparable effectiveness in our patient population.

Note: Proper candidacy screening with drug-induced sleep endoscopy (DISE) and advanced titration are essential for optimal outcomes. Dr. Jain's expertise in both ensures you receive the highest standard of care.

Insurance & Cost

Insurance Coverage

Genio is covered by Medicare and most major commercial insurance plans when FDA candidacy criteria are met (same as Inspire). Coverage typically includes:

  • Pre-operative diagnostic testing
  • Drug-induced sleep endoscopy (DISE)
  • Surgical implantation
  • Post-operative titration and supplies (patches)

The Approval Process

We coordinate all documentation with the surgeon's office, who handles prior authorization and benefit verification for the surgery. The process typically takes 2-4 weeks. Genio and Inspire have identical coverage criteria in most insurance policies.

Patch Costs: Genio patches are disposable (worn nightly) and covered by most insurance plans as durable medical equipment (DME). Out-of-pocket costs vary by plan.

Why Choose a Board-Certified Sleep Medicine Physician for Genio Therapy

Medical Expertise Matters for Sleep Apnea Surgery

Genio implantation requires comprehensive medical evaluation, surgical coordination, and long-term therapy management. Dr. Jain is a board-certified sleep medicine physician trained at Stanford University with expertise in all sleep disorders, not just sleep apnea. This comprehensive training ensures you receive proper candidacy screening, surgical oversight, and ongoing medical management.

Sleep Medicine Physician Provides:

  • Comprehensive sleep disorder diagnosis: Rule out narcolepsy, insomnia, RLS that may coexist with OSA
  • Advanced titration protocols: In-lab titration studies to optimize device settings (many practices cannot perform this)
